Responsibilities include managing client caseloads, completing evaluations, developing treatment plans, documenting to the EHR, and coordinating with community resources to ensure timely discharge planning and follow-up.

Competenze

• Be eligible for Counselor I/II/III roles per PA state requirements.
• Bachelor's degree with the specified years of clinical experience.
• Acceptable degrees include Social Work, Psychology, Addiction, Counseling, Human Services, Criminal Justice.

Mansioni

• Be responsible for a caseload of clients per program requirements and state regulations.
• Complete required evaluations and assessments (screening, intake, clinical assessments, ASAM, biopsychosocial histories).
• Develop individual treatment plans upon client admission.
• Document all clinical services and interventions in the EHR and complete monthly treatment reports.
• Maintain accurate records with timely completion within 24–72 hours of service delivery.
• Conduct individual and group sessions based on treatment plans and level of care.
• Participate in funding source reviews and advise on treatment lengths of stay.
• Monitor client progress and adjust treatment plans as needed.
• Coordinate with agency and community resources for timely treatment planning.
• Assist clients with discharge/continuing care planning and connecting to resources.
• Follow up post-discharge with regulatory guidelines until established with other services.
