Gaudenzia in Erie, PA is seeking counselors to manage a client caseload, conduct assessments, and develop treatment plans in a recovery-focused program. The role emphasizes documentation, timely task completion, and coordination with community resources to support discharge planning.

Competenze

• Associate's degree with two years clinical experience or Bachelor's with one year.
• Counselor II requires Associate's with three years or Bachelor's with two years.
• Counselor III may need Bachelor's with three years or Master's with limited/no experience as specified.

Mansioni

• Be responsible for a caseload of clients, as identified by program requirements and in accordance with state regulations.
• Complete required evaluations and assessments, including screening, intake, clinical assessments, ASAM and biopsychosocial histories.
• Complete individual treatment plans upon client's admission.
• Document all clinical services and interventions performed within the client's electronic health record and complete monthly treatment reports.
• Maintain accurate electronic health records on each client and ensure the timely completion of tasks, within 24 hours of service delivery, no later than 72 hours.
• Conduct individual and group sessions based on treatment plan goals and level of care.
• Participate in funding source reviews and make recommendations for treatment and lengths of stay.
• Monitor client progress and adjust treatment plans as needed.
• Coordinate with agency and community resources to ensure a through treatment plan.
• Assist clients with developing a discharge/continuing care plan and connecting to resources.
• Follow up with clients after discharge according to regulatory guidelines until established with other services.

Formazione

Associate's Degree with two (2) years clinical experience

Bachelor's degree with one (1) year of clinical experience

Bachelor's degree with three (3) years clinical experience; or Master's degree in Social Work, Psychology, Chemical Dependency, Counseling, or Nursing with no experience required; or Master's degree in related field with one (1) year of clinical experience
