Job summary

Orlando Health Bayfront Hospital is seeking a Care Coordinator, Acute Social Worker II to join the Case Management team in St. Petersburg, FL. The role is a PRN pool position with day shifts and weekend/holiday rotations to support coverage.

You will work with a clinical team to identify patients who benefit from care coordination, lead discharge planning, and ensure seamless transitions across inpatient, emergency, and outpatient settings.

Qualifications

• Master’s degree in Social Work, Mental Health, Psychology or Marriage and Family Therapy is required.
• Two (2) years of direct clinical experience with an emphasis on the population to be served; internship may substitute.
• Handle with Care (HWC) Certification required for Behavioral Health Unit.

Responsibilities

• Leads continuity of care across inpatient, emergency and ambulatory settings with integrated discharge planning.
• Builds working relationships with Patient and Family Counselors/Social Workers and UR nurses to advocate and problem-solve.
• Monitors progress toward discharge plans and adjusts plans for high-risk patients.
• Educates patients and families with chronic illness on evidence-based self-management strategies.
• Identifies support needs and develops action plans to initiate and overcome self-management barriers.
• Educates patients and families about the health care system and facilitates cross-setting relationships.
• Ensures access to prescriptions, DME, and other services as needed.
• Participates in problem solving, data collection, and tracking outcomes of treatment options.

Education

Master’s degree from an accredited school of Social Work, Mental Health, Psychology or Marriage and Family Therapy
