Job summary

HomeFirst is seeking a Temp Case Manager for Supportive Services for Veteran Families (SSVF) in the San Jose area. The role focuses on housing stability planning, client-centered services, and coordination with partners to prevent and end homelessness.

This temporary, non-exempt position supports the Program Manager and works Mon-Fri day shift. The Case Manager will document activities in HMIS, maintain records, and participate in team meetings while managing multiple clients and time

Qualifications

• 2 years of paid or volunteer experience in social service or related human services (required).
• Bachelor of Arts in Social Work (BASW) or related human services degree preferred, not required.
• Bilingual Spanish or Vietnamese speaking preferred, not required.
• Strong written and oral communication skills.
• Ability to work with people of diverse socio-economic backgrounds and perspectives.

Responsibilities

• Provide case management and direct services to clients in Santa Clara County.
• Deliver community-based case management services tailored to each client’s needs.
• Coordinate with other team members remotely and on-site as needed to communicate interventions and link clients to additional services.
• Collaborate with community partners to ensure access to support services for clients.
• Develop and implement individualized case management action plans with clients to achieve goals.
• Assist in facilitating life skills workshops when present at program sites.
• Meet with program participants at frequencies determined by their needs and document progress in HMIS.
