Qualifications

Responsibilities

• Assess individuals’ hopes and goals and provide supports to help achieve them.
• Assist with community participation and advocacy while protecting rights.
• Provide daily living support including housekeeping, meals, and transportation as needed.
• Document goal progress, incidents, timesheets, and daily notes.
• Facilitate safety training and connect individuals to community resources.
