Qualifications

• High school diploma required.

Responsibilities

• Assess people for hopes, dreams, and desires and provide supports to help achieve them.
• Assist people in participating in their community according to their interests.
• Protect and promote the rights of people receiving services.
• Assist with advocacy efforts and using personal advocates.
• Provide emotional, physical, and social support as needed.
• Foster positive, person-centered relationships and promote self-determination.
• Contribute to assessment/planning processes and develop individual plans.
• Complete required documentation and progress notes.
• Assist with day-to-day home operations, meals, laundry, and transportation as needed.
• Ensure safety procedures and emergency knowledge are understood.
• Connect people with community resources and life skills services.
• Train and orient new staff; overnight sleepover may be required.
• Attend meetings and maintain training compliance.
