Job summary

hcaoa-careers is seeking a Registered Nurse to provide home health visits in Oklahoma. The role requires an active RN license, CPR certification, and experience in home health or acute care settings. You will conduct assessments, develop care plans, and educate patients and families to optimize independence.

Compensation is paid per visit. A valid driver’s license and reliable transportation are essential, with opportunities to choose visit times to fit your schedule.

Qualifications

• Active and unrestricted RN license in Oklahoma.

• Current CPR certification.
• Must have valid driver’s license and reliable transportation.

Responsibilities

• Complete comprehensive initial and ongoing nursing assessments.
• Perform supervisory visits of home health aides to ensure care plans are followed.
• Develop and evaluate client care plans and revise as needed.
• Maintain accurate EMRs for each visit for billing and regulatory compliance.
• Instruct patients and caregivers on disease management, safety improvements, and self-care.

Duties Include:

• Develop and evaluate client care plans; revising as needed for unmet goals and changes in condition.
• Maintain accurate, timely electronic medical records (EMR) for each visit to comply with billing and regulations.
• Instruct patients and caregivers on disease management, safety improvements, and self-care techniques to promote independence.
• Communicate with family, caregiver and the team any changes in the patient' s physical or emotional condition.
• Participates in medical record audit as assigned.
