Job summary

Home Helpers of South Tulsa is seeking a Registered Nurse (RN) to manage patient care in home environments. Responsibilities include conducting nursing assessments, supervising aides, and developing care plans. Qualifications

• Licensure: Active and unrestricted Registered Nurse (RN) license in Oklahoma.
• Experience: Previous experience in home health or acute care (ICU/ER/Med Surg) required.
• Transportation: Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Complete comprehensive initial and ongoing nursing assessments.
• Perform supervisory visits of home health aides to ensure care plans are being followed.
• Develop and evaluate client care plans, revising as needed.
• Maintain accurate electronic medical records (EMR) for each visit.
• Instruct patients and caregivers on disease management and self-care techniques.
• Communicate changes in the patient’s condition with family and caregivers.

• Communicate with family, caregiver and the team any changes in the patient’s physical or emotional condition.
• Participates in medical record audit as assigned.
