Job summary

A Path of Care Home Health in Weatherford, OK is seeking a PRN Registered Nurse (RN) to support our Weatherford team. This role offers meaningful work, flexibility, and an opportunity to grow your nursing career in patients' homes.

As a PRN RN, you will conduct patient visits, develop care plans, provide skilled nursing treatments, educate patients and caregivers, coordinate with physicians, and help with discharge planning. The position requires Oklahoma RN license and CPR.

Qualifications

• Active Oklahoma RN license.
• Minimum 1 year nursing experience.
• CPR certification (or ability to obtain within 60 days).
• Valid driver's license, reliable transportation, and proof of insurance.
• Ability to travel long distances and adapt to changing schedules.
• Experience in home health, critical care, or emergency care is preferred.

Responsibilities

• Conduct patient visits to assess nursing needs.
• Develop individualized plans of care from initial evaluations.
• Perform ongoing assessments and revise plans under physician direction.
• Provide skilled nursing treatments and educate patients and caregivers on disease processes, self-care, and prevention.
• Initiate preventive and rehabilitative nursing procedures as ordered.
• Prepare clinical and progress notes and coordinate services with the care team.
• Communicate with physicians and staff to ensure continuity of care.
• Re-evaluate patient needs and assist with discharge planning.
• Supervise LPNs, vocational nurses, and home health aides as appropriate.
• Practice infection control and universal precautions.
• Report suspected abuse per state and federal guidelines.

A Day in the Life of a Registered Nurse (RN)

As a PRN RN, you play a vital role in assessing and supporting patients in their homes. Your responsibilities include:

• Conducting patient visits to assess nursing needs
• Completing initial evaluation visits and developing individualized plans of care
• Performing ongoing assessments and revising plans as needed under physician direction
• Providing skilled nursing treatments and educating patients and caregivers on disease processes, self‑care, and prevention
• Initiating preventive and rehabilitative nursing procedures as ordered
• Preparing clinical and progress notes and coordinating services with other care team members
• Communicating directly with physicians and staff to ensure continuity of care
• Re‑evaluating patient needs and assisting with discharge planning
• Supervising LPNs, vocational nurses, and home health aides when appropriate
• Practicing infection control and universal precautions
• Reporting suspected abuse according to state and federal guidelines
• Assisting with office tasks and new‑nurse orientation when needed
