Qualifications

• BSN or ADN required with plan to complete BSN within 5 years if enrolled in ADN.
• RN license or eligibility to practice in Virginia.

Responsibilities

• Performs comprehensive patient health assessments.
• Forms individualized plans of care based on assessments.
• Coordinates care transitions with other disciplines.
• Makes clinical decisions using experience and data.
• Educates patients and families; documents outcomes.
• Provides clinical guidance and mentors other staff.
• Adheres to standards and pathways for care delivery.
• Maintains continuing education records and participates in quality initiatives.
