Job summary

The Link & Option Center, Inc. in South Holland, IL seeks an IM+CANS Assessor to perform assessments and evaluations using IM+CANS and other tools.

You will complete documentation in the EHR and collaborate with clinical staff to assure appropriate services and care levels. Responsibilities include completing IM+CANS within policy timelines, documenting and submitting notes, and providing safety planning and crisis education to clients.

Responsibilities

• Complete IM+CANS with policies and procedures within specified timelines.
• Review initial diagnoses and treatment recommendations.
• Document in the client's EHR (Dr. Cloud).
• Submit encounter notes within policy timelines.
• Consult with the Director of Clinical Operations on differential diagnoses.
• Coordinate with Intake Staff to link clients to appropriate services.
• Provide consultation to assigned clinical staff on assessment and treatment.
• Create Safety Plans with clients when needed and arrange follow-up.
• Educate all clients about the 24-hour Crisis Hotline.
• Engage clients to complete Crisis Plans.

JOB TYPE: Full-Time

The Link & Option Center, Inc. is a Joint Commission–accredited outpatient State Certified Community Behavioral Health Clinic (CCBHC) serving the far Southside community and designated to serve the Southland region with locations in South Holland and Hazel Crest, Illinois. Job Summary:

The Assessment/Evaluator is responsible for completing the IM+CANS and other evidence-based assessment tools as indicated by the client’s presenting problems.

RESPONSIBILITIES:

• Complete IM+CANS with TLOC policies and procedures (i.e., within 1 day of urgent care and within 4 days for routine care).
• Reviews and confirms initial diagnosis and treatment recommendations.
• Completes required documentation in the client’s EHR Dr. Cloud.
• Completes and submits encounter notes as required by TLOC policy (i.e., within 45 hours).
• Confers with the Director of Clinical Operations regarding questions related to differential diagnosis.
• Works closely with Intake Staff to ensure that clients are linked to appropriate services and level of care.
• Provides consultation to assigned clinical staff regarding assessment and treatment recommendations.
• Completes a Safety Plan as needed with the client and provides appropriate follow-up.
• Ensure at minimum, all clients must be educated about 24-hour Crisis Hotline.
• All clients are engaged to complete Crisis Plan.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Must meet criteria for qualification as a QMHP (i.e. master’s degree in a human services-related field).
• State of Illinois IM+CANS certified.
• Working knowledge of DSM V diagnosis.
• High degree of professionalism in dealing with diverse groups of people, including clients, staff, and others.
• Demonstrated ability to work collaboratively with other departments within TLOC.
• Ability to make appropriate, informed decisions.
• Ability to multi-task, complete a high volume of tasks and projects with little or no guidance.
• Well-developed organizational and time management skills.
• Effective verbal and written communication skills.
• Professional discretion and adherence to confidentiality.
• Must be culturally competent, empathetic, and able to interact and develop rapport with a diversified population.
