Job summary
Inova Fairfax Hospital is seeking a dedicated Transplant Coordinator to join the Advanced Lung Disease team. The coordinator will manage ILD patient care across outpatient clinics, coordinating diagnostics, multidisciplinary reviews, medication access, and transplant/referral pathways, with emphasis on timely testing, education, and rapid escalation when clinically needed.
The role is full-time, day-shift, Monday through Friday, with rotational remote on-call (about 3 weekends per year and 2
Qualifications
- Bachelor's degree in nursing required.
- Licensed in Virginia as a Registered Nurse; BLS certification.
- Minimum of 3 years RN experience.
Responsibilities
- New patient intake and pre-visit planning for ILD patients.
- Coordinate ILD workups, referrals, and multidisciplinary reviews.
- Monitor and manage oxygen, medications, and testing timelines.
- Educate patients on ILD, disease progression, and self-management.
- Triage urgent calls and coordinate appropriate escalation.
