Job summary

Inova Fairfax Hospital in Northern Virginia is seeking a dedicated Transplant Coordinator to join the Advanced Lung Disease team. The Transplant Coordinator will coordinate care for patients with interstitial lung disease across outpatient clinics, including diagnostics, multidisciplinary review, medication access, and transplant/referral pathways.

The role is full-time, day-shift, Monday–Friday with no weekends or holidays.

Qualifications

• Bachelor’s degree in nursing
• Three years of RN experience
• Licensed in the Commonwealth of Virginia as a Registered Nurse; Basic Life Support (AHA)

Responsibilities

• Coordinate new patient intake and pre-visit planning including outside records and urgent needs
• Coordinate ILD diagnostic workups and referrals per protocol
• Monitor symptoms, oxygen needs, and surveillance testing over time
• Educate patients on ILD, disease course, vaccines, energy conservation, and oxygen safety
• Facilitate antifibrotic therapies and prior authorizations, and manage lab monitoring
• Arrange home oxygen and DME coordination for payer recertification
• Triage calls for worsening symptoms and escalate to provider/ED when needed
• Coordinate referrals to pulmonary rehab, nutrition, social work, and palliative care
• Identify transplant referral indicators and assemble required records for transfer
• Document care coordination and maintain open communication within teams
• Support quality initiatives and workflow optimization
