Job summary

Easterseals NH is seeking a Special Education Case Manager for its Gammon Academy in Manchester, NH. The role coordinates treatment plans, collaborates with teachers and related services to develop IEPs, and monitors progress for youth with disabilities.

Qualifications include a Bachelor’s in Special Education with certification; Master’s degree preferred and 3–5 years of experience; valid driver’s license and reliable transportation are required. Full-time, year-round position with benefits.

• Master's degree in Special Education preferred.
• Certification in Special Education required.

Responsibilities

• Coordinate and implement all aspects of the treatment plan for all assigned students.
• Coordinate with teachers and related services to create IEPs and ensure plans are followed.
• Assist in assessment of each youth and monitor progress.
• Facilitate team meetings and conferences and attend meetings as required.
