South Mississippi Regional Center, a program of the Mississippi Department of Mental Health, seeks a Social Services Program Coordinator III (Targeted Case Manager) for Long Beach. You will manage IDD Community Support Program caseloads and develop person-centered Plans of Services and Supports.

The role requires a Bachelor's degree in psychology or a related field and 4–7 years of experience, with travel across six southern counties and a valid Mississippi driver’s license.

Qualifikationen

• Bachelor’s degree in psychology or related mental health field required.
• 4–7 years of related experience preferred.
• Ability to travel within the six southern counties and maintain a state vehicle license.

Aufgaben

• Serves as IDD Targeted Case Manager (TCM) for people enrolled in the IDD Community Support Program (1915i).
• Manages and maintains caseload of people receiving IDD Community Support Program services.
• Develops and maintains a person-centered Plan of Services and Supports (PSS) for each person on caseload.
• Revises/updates each person’s PSS at least annually or when changes in the person’s circumstances occur or when requests are made by the person/legal representative.
• Coordinates and monitors identified supports and services through regular monthly contact, face to face quarterly reviews, and annual assessment and re-certification meetings.
• Ensures services are adequate, appropriate, and meet the person’s needs.
