Job summary

Georgia Department of Behavioral Health and Developmental Disabilities is seeking a State Services Coordinator for the Region 3 Field Office in Atlanta. The role is field-based, collaborating with individuals with IDD to develop and monitor ISP plans, coordinate services, and support families across multiple counties.

Ideal candidates will hold a Bachelor's degree in a related field and have at least two years of related experience; Spanish fluency is preferred.

Qualifications

• Bachelor's degree in a related field.
• Two (2) years of job-related experience.
• Licensure eligibility may be required for some positions.

Responsibilities

• Communicate with individuals and families to gather information and monitor changes in needs and services.
• Assist individuals with IDD in selecting state-funded service providers and facilitate ISP meetings.
• Develop and maintain Individual Support Plans (ISPs) and reassess needs regularly.
• Coordinate community resources and liaise with providers, families, and caregivers.
• Ensure compliance with HIPAA and DBHDD confidentiality policies; report concerns as required.
