Qualifications

• A bachelor’s or advanced degree from an accredited college or university preferred
• OR an associate degree in a social, behavioral, human service, or health-related field and one year of paid or unpaid experience with individuals with intellectual or developmental disabilities
• OR a high school diploma or equivalent plus two years of paid or unpaid experience with individuals with intellectual or developmental disabilities
• Must have reliable transportation and a valid driver’s license
• Work experience in a LIDDA or IDD provider services preferred

Responsibilities

• Coordinate services for individuals on caseload and ensure paperwork is up to date
• Communicate effectively with families and stakeholders
• Staff individuals served weekly with leadership feedback in a collaborative team
• Promote trauma-informed, person-centered approaches in all interactions
• Link individuals to additional resources to support goals and navigate services
• Troubleshoot and organize support, interventions, and services to achieve outcomes
• Maintain boundaries while improving outcomes for individuals served and families
