Job summary

Bluebonnet Trails Community Services is seeking a Service Coordinator to deliver person-centered assessments and develop comprehensive service plans for children and adults with developmental disabilities in Bastrop and Lockhart, TX. The role requires traveling within the service area and flexible hours.

Responsibilities include coordinating services, advocating for individuals, and ensuring compliance with Medicaid, HHSC, MCOs, and BTCS guidelines.

Qualifications

• Associate degree or higher in social, behavioral, or human service field.
• High school diploma or equivalent with two years of relevant experience with individuals with intellectual or developmental disabilities.
• Must meet driving requirements and maintain driving status throughout employment.

Responsibilities

• Conduct comprehensive assessments of individual needs and desires.
• Develop service plans and facilitate service linkages.
• Monitor services and advocate on behalf of individuals served.
