Job summary

Breckenridge Village in Texas seeks an experienced health and human services supervisor to lead direct resident care, coach staff, and ensure compliance with care plans. You will document in PCC, support independence, and coordinate with the interdisciplinary team.

A strong background in IDD care is preferred. The role requires at least 2 years in HHS with direct client care; an Associate or higher degree is preferred.

Qualifications

• Associate degree or relevant certifications with experience.
• Preferred: Associate or Bachelor in Human Services or HHS certification with 2+ years experience.
• Experience in Health and Human Services with direct client care.

Responsibilities

• Provide direction, coordination and support to AHM, ensuring smooth handoffs and consistent care.
• Assist residents in achieving active treatment goals and document in PCC.
• Act as lead staff in the home, supporting routines and care plans.
• Promote independence and meaningful choices for residents.
• Assist with hygiene, grooming, laundry, housekeeping, and meal prep.
• Transport residents per BV protocols and attend interdisciplinary meetings.
• Maintain home safety, documentation, and timely reports.
