Job summary

BV in Texas seeks a Housing/Residential Services AHM to provide direct client care for residents, with a focus on active treatment goals, documentation in PCC, and coordination with an interdisciplinary team.

The role requires at least 2 years in Health and Human Services, an Associate's degree (or related certification), strong communication and mentoring skills, and ability to manage schedules, budgets, and safety drills. On-site apartment benefits are included.

Qualifications

• Associate degree or HHS certification with experience
• 2+ years in Health and Human Services role
• Experience supporting individuals with IDD preferred
• Leadership or supervisory experience preferred

Responsibilities

• Provide direction, coordination and support to AHM and maintain consistent care
• Assist residents in achieving active treatment goals and document in PCC
• Lead staff in the home and support routines, health care needs and care plan implementation
• Promote independence and offer meaningful choices to residents
• Assist with hygiene, grooming, laundry, housekeeping and meal prep using menus
• Transport residents to events and appointments per transportation protocols
• Participate in interdisciplinary team meetings and care planning sessions
• Maintain home cleanliness and safety; submit maintenance requests
• Conduct safety and emergency drills and ensure compliance with policies
