General Description

The purpose of this job is to provide a continuum of care for individuals with IDD in the least restrictive environment. The scope of this position is to work collaboratively with LIDDA Service coordinators, IDD HCS & TxHmL public and private providers, Crisis teams, Habilitation coordinators, Enhanced community coordinator, LMHA, NF staff, SSLC staff, and external stakeholders. Expectations and requirements will follow the Local Intellectual and Developmental Disability Authority (LIDDA) Performance contract with Health and Human Services (HHS).

This class works under close to general supervision according to set procedures but determines how or when to complete tasks.

Duties and Responsibilities

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position, as necessary.

• Provide coverage for evaluator responsibilities during staff vacancies or as needed.
• Provide information and assistance to LIDDA staff who are facilitating diversion for people at risk of admission to a nursing facility (NF) and for people transitioning to the community from an NF.
• Identify available community living options, services, and supports to assist individuals to successfully live in the community.
• Within 45 to 75 calendar days after a person is admitted into an NF, review the person's admission to ensure that community living options, services, and support that could provide an alternative to NF services have been explored and, if not, refer the person to their rehabilitation coordinator.
• Identify, arrange, and coordinate access to community services as a diversion to NF admission for a person who has chosen community living during the PE process.
• Request a targeted NF Home and Community-based Services (HCS) diversion slot, targeted NF HCS Diversion Slot, or transition slot.
• Conduct Intakes, as applicable, according to targeted NF Home and Community-based Services (HCS) diversion slot requests.
• Initiate and complete the process for State supported living center.
• Participates in TST Multidisciplinary case consults meeting on a recurring basis and provides input and suggestions within scope of expertise.
• Participates in TST drop-in hours on a recurring basis and provides input and suggestion within scope of expertise.
• Conducts outreach in the community to establish rapport with IDD providers and external stakeholders.
• Identify available community living options, services, and supports to assist individuals to successfully live in the community.
• Attends all team and individual meetings as required by supervisor and maintains open communication regarding assigned caseload or such matters that affect the delivery of services to all clients.
• Demonstrates the ability to organize and structure days and times with effective time management skills to successfully fulfill all essential duties.
• Completes all program required forms in accordance with HHSC and internal timeframes and ensures all progress notes are entered within 48 hours at a rate of 100%.
• Shows dedication to ethical judgment and maintains compliance with all training requirements to effectively accomplish position duties and meet performance outcomes and targets.
• Performs other duties as assigned.

Minimum Education and Experience Requirements

Requires a Bachelor's Degree with a major in a social, behavioral, or human service field including, but not limited to psychology, social work, medicine, nursing, rehabilitation counseling, sociology, human development, gerontology, educational psychology or special education supplemented by one (1) year of experience in a community setting; or possession of any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities.

Experience with trauma-informed services; cognitive behavioral therapies, including Dialectical Behavioral Therapy (DBT); and motivational therapies including the use of incentives, preferred.

Required Knowledge and Abilities

Knowledge of trauma-informed theories, principals, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred.

Physical Demands

Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

Special Certifications and Licenses
