Job summary

NorthKey Community Care is seeking a full-time Case Manager for its Intellectual and Development Disabilities (IDD) Crisis Program. The role monitors crisis support services and facilitates stabilization for IDD clients.

Qualifications

• Bachelor's degree in a human services field is required.
• At least one year of experience working with IDD clients is required.
• Excellent written and verbal communication skills are required.

Responsibilities

• Monitor crisis support services and facilitate stabilization of crises for IDD clients.
• Collaborate with the IDD Crisis Services Team Leader to develop mobile crisis response for IDD individuals.
• Provide input on plan of care with family, guardian, and providers with client consent.
• Complete documentation and reports and ensure HIPAA compliance.
