Job summary

Qualifications
- Bachelor's degree from an accredited education program in a human services field, or a bachelor's in a non-human services field with two years of human services experience.
Responsibilities
- Provide Case Management services to individuals with intellectual and/or developmental disabilities.
- Establish and monitor a Person Centered Plan (PCP) and PCP updates.
- Advocate for individuals, educate families, and coordinate services with external providers.
- Identify ongoing support services and secure resources aligned with individualized plans.
- Travel as required to meet with individuals and support staff, within community settings.
