Job summary

Qualifications

• Bachelor's degree with two years of relevant experience, or a license as a Registered Nurse with two years of relevant experience, or a master's degree with one year of relevant experience.
• Fluent understanding of community cultural traditions, norms, and practices of members and their families.
• Absolute integrity and commitment to serving people with I/DD and their families.
• Excellent interpersonal, public speaking, and written communication skills.

Responsibilities

• Deliver person-centered care management services in compliance with regulatory standards and agency policies.
• Complete a comprehensive assessment/reassessment process.
• Identify gaps in service and make referrals; advocate for members to achieve goals.
• Develop, implement, and monitor Life Plans with an interdisciplinary team.
• Ensure timely documentation and accurate billing-related records.
• Travel throughout the designated service area to meet members and providers.
