RHA Health Services, LLC is seeking an IDD Care Manager for Wake & surrounding counties. This community-based hybrid role requires travel and offers flexible schedules.

Job Overview

We are hiring for: IDD Care Manager Wake & Surrounding Counties Type: Regular. This hybrid, community‑based position requires travel within and surrounding the county. We offer flexibility in the schedule structure.

Responsibilities
  • Foster and facilitate the care management process.
  • Conduct Care Management Comprehensive Assessments, share information and facilitate Tailored Care Management Planning with the person supported and other team members.
  • Monitor the ongoing care management plan implementation and facilitate Care Plan Updates every 12 months minimally or when there is a significant change of circumstance or need or at the request of the person supported.
  • Implement workflows, policies and procedures, and documentation standards that promote whole‑person care.
  • Maintain a positive and professional image in the work setting and community.
  • Follow policies and procedures to ensure accountability of service delivery.
  • Demonstrate a passion for positive change by continuously improving and defining innovative care management interventions. Stay up to date with current best practices and keep the team continuously informed about best practice innovations.
  • Collaborate cross‑functionally with other Integrated Care Team members, community organizations and RHA departments to ensure success of Care Management implementation.
  • Maintain a person‑centered philosophy focusing on an individual’s strengths, needs, and preferences that promote health, wellness, and safety. Foster a person‑centered culture with Care Management Teams to optimize individual education around self‑management, disease management, and integrated healthcare planning.
  • Implement the strategic use of data to support delivery of effective, integrated, and well‑coordinated care management services. Ensure individual data sharing and storage consistent with state and company policies.
  • Implement all organization health and safety policies and procedures to maintain a Safe and Healthy Environment.
  • Document services provided in a timely manner to ensure appropriate account for reimbursement purposes.
  • Establish and maintain frequent and regular communication with persons supported, family members, other team members and colleagues. Foster open communication and feedback. Help maintain focus on care planning goals and outcomes.
  • Establish and maintain strong external relationships, including providers, community organizations, and Clinically Integrated Networks (CIN).
  • Support and participate in an ethical culture in conjunction with the RHA Code of Conduct and Ethics and Compliance Program, emphasizing confidentiality and no retaliation policies. Direct the confidential and immediate investigation into allegations of abuse, neglect, exploitation, theft, fraud, waste, misconduct, and violations of ethics and compliance practices according to company policy. Ensure protections of individuals served during investigations and follow all external reporting requirements, ensuring adequate follow‑through on recommendations from investigation reports.
  • Participate as assigned as a designated member to Quality Improvement Committee, Quality Assurance Committee, Quality Management Team, Safety, Task Force and Human Rights Committee meetings.
  • Perform all other related duties as assigned.
Qualifications
  • Education and Experience:
    • For all populations served: Care Managers must meet at least ONE of the following qualifications to be a Qualified Professional (QP), per 10A NCAC 27 .0104:
      • Master’s degree in a human service field ** AND at least one year of full‑time experience working with the MH/SU/IDD population *
      • Bachelor’s degree in a human service field ** AND at least two years of full‑time experience working with the MH/SU/IDD population *
      • Bachelor’s degree in a non‑human service field AND at least four years of full‑time experience working with the MH/SU/IDD population *
    • For care managers serving members with LTSS needs: Must meet at least one of the above qualifications AND two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience (this experience may be concurrent with the years of experience required to become a Qualified Professional).
  • Other requirements:
    • Valid driver’s license. Clinical license preferred. Reliable transportation.
  • Pre‑employment screening: Complete criminal background name checks in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities), drug testing, education verification and other credentialing based on position requirements, proof of employment history or references (if required). Positions that require driving must provide proof of driver’s license, driver’s insurance, and vehicle if required for providing transportation for individuals.
