RHA Health Services, LLC is hiring a Care Manager for adult and child behavioral health serving Wake County. This is a hybrid community-based role requiring travel within the county and surrounding areas.

You will conduct comprehensive assessments, develop Tailored Care Plans, coordinate with teams, and monitor plan implementation to improve health outcomes. Qualifications include a master’s or bachelor’s in a human service field with relevant experience; valid driver’s license and reliable

Qualifications

• Master’s degree in a human service field with at least one year of MH/SU/IDD experience.
• Bachelor’s in a human service field with at least two years of MH/SU/IDD experience.
• Bachelor’s in a non-human service field with at least four years of MH/SU/IDD experience.
• LTSS/HCBS coordination experience in addition to general care management experience.
• Valid driver’s license and reliable transportation; clinical license preferred.

Responsibilities

• Conducts Care Management Comprehensive Assessments and tailors Care Plans with the person supported and team.
• Monitors care plan implementation and updates every 12 months or as needs change.
• Collaborates with Integrated Care Team and community partners to support care management.
• Maintains data-informed practices and ensures documentation standards.
• Ensures safety, ethics, and compliance in all activities.
