Job summary

3M’s Services LLC is seeking an experienced Qualified Professional to join our growing team in North Carolina. The QP will manage a caseload of individuals receiving community-based services and ensure person-centered, compliant support aimed at independence and community involvement.

The role requires collaboration with families, Care Managers, and direct support staff, with a hybrid schedule, on-site training, and field work. A relevant degree and supervised MH/DD/SA experience are essential.

Qualifications

• Master's degree in a human services field with at least one year of qualifying supervised MH/DD/SA experience
• Bachelor's degree in a human services field with at least two years of qualifying supervised MH/DD/SA experience
• Bachelor's degree in a non-human services field with at least four years of qualifying supervised MH/DD/SA experience
• An applicable professional license, certificate, registration, or permit with the qualifying experience required for the population served

Responsibilities

• Manage and oversee an assigned caseload of individuals receiving community-based services
• Coordinate and monitor services according to ISP/Plan of Care and authorized services
• Participate in ISP meetings, treatment team meetings, and other planning meetings
• Maintain regular communication with individuals, families/guardians, Care Managers, and leadership
• Complete required monitoring and community/site visits
• Monitor goals, progress, service utilization and quality of services
• Supervise, coach, and support Direct Support Professionals assigned to your caseload
• Conduct staff observations and required supervision visits
• Train staff on goals, support needs, and documentation expectations
• Review service documentation for accuracy and timeliness
• Monitor authorizations, consents, assessments and other required documentation
• Monitor EVV compliance for applicable services
• Identify staffing or service-delivery barriers and collaborate to resolve them
• Assist with interviewing, onboarding, and matching DSPs with individuals
• Ensure services meet Medicaid, LME/MCO, state, and agency requirements
• Identify and communicate health, safety, service, or compliance concerns to the Program Director
• Participate in audits, quality improvement activities, staff meetings, and required trainings
• Maintain organized, audit-ready records for assigned cases
