Job summary

The Wright Center Infectious Disease Clinic is seeking a Licensed Practical Nurse (LPN) to support patient-centered care in an outpatient setting. The LPN will assist with monitoring, medication administration, vaccinations, lab coordination, and pre-visit planning under supervision of the ID Physician and clinic leadership.

Responsibilities include outreach, care coordination across the care continuum, documentation in the EMR, and participation in quality improvement initiatives for HIV,

Qualifications

• Current PA Licensed Practical Nurse (LPN)
• Active BLS certification required
• Strong patient communication and care coordination skills
• Knowledge of EMR systems
• Experience in ambulatory or primary care settings preferred
• Experience with vaccinations, injections, and patient education preferred

Responsibilities

• Conduct outreach calls for appointments, lab testing, medications, screenings, vaccinations, and follow-up care
• Reach patients overdue for appointments or services and re-engage those out of care
• Document all patient communications in the EMR
• Coordinate scheduling, labs, vaccinations, and follow-up tests
• Administer medications, injections, and immunizations as ordered
• Monitor patient vitals and health status during visits and transitions of care
• Support vaccine administration, inventory, and documentation
• Identify care gaps and communicate with providers and care team
• Maintain patient tracking tools and quality measures dashboards
