Qualifications

• Current Pennsylvania LPN license is required.
• Active BLS certification is required.
• Minimum 1–3 years of ambulatory or primary care experience; infectious disease or specialty clinic experience preferred.

Responsibilities

• Conduct outreach calls for appointments, lab testing, vaccinations, and follow-up care.
• Assist in re-engagement of patients out of care and coordinate scheduling.
• Document all patient communications in the electronic medical record.
• Coordinate laboratory scheduling and follow-up testing and notify patients of results.
• Administer medications, injections, immunizations, and other treatments as ordered.
• Support HIV, Hepatitis C and infectious disease related care coordination and quality improvement initiatives.
