Responsibilities

• Assesses patient and completes admission forms per hospital policy.
• Assesses and diagnoses patient and family needs to provide quality care to assigned patients.
• Develops, discusses, and communicates a realistic plan of care with patient and support person.
• Implements medical plan of care and nursing interventions as necessary.
• Evaluates problems and patient responses to ensure quality care.
• Maintains up-to-date documentation for the healthcare team.
• Performs other duties as assigned.
