• Assesses and diagnoses patient and family needs to provide quality care.
• Develops, discusses, and communicates a realistic plan of care with patient and support person.
• Implements nursing plan of care and procedures as necessary for patient care.
• Evaluates problems, care provided, and patient responses to ensure quality care.
• Maintains accurate documentation of nursing care for the care team.
