Qualifications

• Education: Program Graduate
• Nonessential: Board Certification
• Education specialization: Accredited College of Pharmacy
• Nonessential: MS Pharm, PharmD, Pharmcotherapy Board Cert w/in 3 Years
• Experience: Pharmacy experience in a community or hospital pharmacy as a licensed pharmacist or intern as determined by review committee
• Credentials: CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
• Pharmacist License in NM w/in 90 days
• NM Chemotherapy Certification (nonessential)

Responsibilities

• Consult with physicians, other health professionals, and patients about drug usage and substitutions
• Compound and dispense medications per physician orders in inpatient, outpatient, and ambulatory settings
• Participate in Continuous Improvement (CQI) program at UNM Hospitals
• Maintain inventory records on narcotics and other controlled substances
• Ensure timely filling and delivery of physician prescriptions or orders
• Prepare and dispense doses of medications as required to meet patient needs
• Review patient profiles for drug interactions and dosing appropriateness
• Participate in medication usage evaluations and create clinical practice guidelines
• Attend department meetings and represent the Hospitals on related committees
• Provide patient counseling and educate patients and families about medications
• Perform quality improvement and pharmacokinetic monitoring tasks as assigned
• Train residents, students, and technicians
