• Coordinate patient care with patients, families, and healthcare providers.
• Provide patient and family education and health information.
• Promote a safe and effective patient care environment.
• Participate in discharge planning to ensure continuity of care.
• Delegate and coordinate duties of healthcare team members.
• Perform other job-related duties as assigned.
