Job summary

AHRC Nassau seeks a supervisory professional to oversee direct support staff delivering person-centered care. The role focuses on monitoring progress, guiding staff, and maintaining safety and quality of life for residents.

The position entails staff onboarding, ongoing training, and coordination of daily living support, with emphasis on safety, incident prevention, and adherence to program plans.

Qualifications

• High school diploma with 6 months experience with I/DD population, or a health-related college degree.
• Valid NYS driver’s license required.

Responsibilities

• Supervise DSP staff in implementing the person-centered program plan and review progress.
• Assist with onboarding new DSP staff and ensure training on program plans and annual trainings.
• Monitor DSP data such as goals, behavior plans, BM charts, mealtime data and repositioning.
• Assist in completing daily assignment sheets and ensure accurate information.
• Ensure community outings and medical appointments are fulfilled daily.
• Provide direct support for residents with daily living skills while ensuring safety.
• Deliver in-service training and feedback to staff to ensure proper performance.
• Attend meetings to evaluate programs and discuss changes when needed.
• Ensure agency policies are implemented and enforced.
• Promote diversity, equity, and inclusion in the workplace.
