Job summary

AHRC Nassau seeks a supervisor to lead direct support professionals in delivering person-centered care, monitor progress, and ensure safety and quality of life for residents. The role includes mentoring new DSP staff, coordinating trainings, and maintaining accurate daily data.

You'll work closely with the ICF Supervisor, supervise daily living activities, assist with scheduling, and support community outings and medical appointments, all while promoting diversity and inclusion across the team.

Qualifications

• High school diploma or better with six months experience with the I/DD population or a health service degree.
• Must have a valid and qualified NYS driver’s license.

Responsibilities

• Supervises DSPs in implementing the person-centered program plan and reviews progress.
• Assists and trains new DSP staff and ensures annual trainings and incident reporting are completed.
• Monitors DSP data (goals, behavior plans, charts, mealtime data) daily and ensures accuracy.
• Helps prepare daily assignment sheets and coordinates community outings and medical appointments.
• Provides direct support with daily living tasks ensuring safety according to plans.
• Delivers in-service training and supports program evaluations and potential changes.
• Upholds agency policies and fosters diversity, equity, and inclusion.
