Millennium Physician Group in Florida is seeking a Compliance Coding Chart Auditor II to independently conduct comprehensive medical chart audits evaluating accuracy, completeness, and regulatory compliance of clinical documentation, ICD-10-CM coding, risk adjustment, and professional medical services billing.

You will perform retrospective and prospective reviews to ensure ICD-10-CM guidelines, HCC requirements, E/M coding, CPT and HCPCS accuracy, and identify unsupported diagnoses or incorrect

Qualifikationen

• Minimum 2–4 years of medical coding and chart audit experience.
• Experience assessing ICD-10-CM codes and HCC risk adjustment.
• Knowledge of CPT/E/M guidelines and HCPCS Level II.
• Ability to communicate audit findings and educate providers.

• Review provider documentation to ensure diagnoses, procedures, services, and codes are supported.
• Determine whether medical records support codes and billed services.
• Evaluate consistency between documentation, codes, and guidelines.
