Job summary

Integrated Services for Behavioral Health (ISBH) is seeking an I-FAST Community Behavioral Health Worker in Noble County, OH. You will engage individuals and families in home and community settings, implement I-FAST interventions, and support youth aged 5-21 to build skills and improve daily functioning.

Education requirements include a high school diploma or GED; a bachelor’s degree in healthcare or behavioral health or three years of relevant experience.

Qualifications

• High school diploma or GED is required.
• Bachelor's degree in healthcare, behavioral health or related field or three years of relevant experience is required.
• Experience working with multisystems (schools, courts, child welfare) is preferred.

Responsibilities

• Engage individuals and families in home and community to identify strengths, barriers and goals.
• Provide direct interventions using the I-FAST model.
• Conduct assessments and inform treatment planning.
• Collaborate with schools, care management, and other teams.
• Provide care in home and community-based settings.
• Ensure continuity of care and 24/7 coverage as needed.
• Adhere to ISBH documentation policies and HIPAA requirements.
