Job summary

Integrated Services for Behavioral Health seeks an I-FAST Community Behavioral Health Worker in New Lexington, OH to engage youth and families in home and community settings, delivering direct interventions under the I-FAST model and coordinating with schools and care teams.

The role requires collaboration with multi-system partners, ongoing training, and adherence to HIPAA and EHR policies. Travel within designated service areas is required.

Qualifications

• High school degree or GED is required.
• Bachelor's degree in healthcare, behavioral health or related field or three years of relevant experience is required.

Responsibilities

• Engage individuals and families in home/community to identify strengths, barriers, and goals related to behavioral health and daily functioning.
• Provide direct interventions using the I-FAST model.
• Conduct treatment planning and collaborate with schools, courts, care management entities, etc.
• Provide care in home and community-based settings.
• Travel within designated service areas.
