Job summary

Integrated Services for Behavioral Health (ISBH) is seeking an I-FAST Community Behavioral Health Worker to support youth and families in home and community settings. You will apply the I-FAST treatment model, coordinate with schools, courts, care management, and community partners to address behavioral health goals.

This role requires 2–3 years healthcare or behavioral health experience, a high school diploma or GED (bachelor's preferred), strong communication and organizational skills, and the

• Advanced communication skills, both oral and written.
• Strong organizational skills and ability to manage deadlines in a fast-paced environment.
• Ability to maintain confidential information and bill Medicaid without restrictions.

Responsibilities

• Engage individuals and families in home and community to identify goals related to behavioral health.
• Provide direct interventions using the I-FAST model and principles.
• Conduct assessments of client and family to inform problem behaviors and ecological context.
• Participate in treatment planning with schools, courts, and care management entities.
• Travel within designated service areas to provide services in homes and community settings.
