Qualifications

• Must be at least 21 years old.
• Must have a High School Diploma or GED.
• Must be able to read, write, understand, and follow directions.
• No substantiated findings on the NC Health Care Abuse Registry.
• Must meet SBI and DMV requirements for contracts.
• Must have a valid driver’s license and 3 years driving experience.
• Must have automobile liability insurance.
• Experience with persons with I/DD preferred but not required.
• Must complete required trainings as assigned.
• Must have internet access at the time services are rendered.

Responsibilities

• Provide one-on-one support to individuals with I/DD to reach personalized goals.
• Instruct daily living skills, socialization, employment, leisure and activities.
• Document services in progress notes daily in BLACK INK.
• Obtain guardian initials/signatures for hours worked and services.
• Adhere to policy preventing unsupervised private home visits without approval.
• Supervise/assist with housekeeping, meals, grooming, bathing, and eating as applicable.
• Provide transportation to community services and activities when required.
• Assist with money management.
• Encourage independent decision making and scheduling.
• Assist with prescribed exercises taught by professionals.
• Perform incidental household tasks essential to care.
• Monitor progress notes and records; submit by deadlines.
• Complete minimum 12 hours of training annually.
• Be familiar with TA, Inc. policies and emergency procedures.
• Attend mandatory staff meetings or notify coordinator if unable.
• Receive on-site supervision monthly or as needed.
• Work under supervision of a Qualified Professional per NCAC rules.
• Remain aware of state regulations for assigned person served.
• Work only service-ordered hours unless approved by Administrator.
• Care for and maintain therapeutic/adaptive equipment.
• Report changes in condition or needs to the appropriate personnel.
