Job summary

People Incorporated Mental Health Services in Duluth, MN, is seeking a Case Manager II for Waiver Case Management. You will develop and implement person-centered support plans, coordinate services, and help clients live as independently as possible within their communities.

The role is full-time with hybrid scheduling after training, supporting clients in St. Louis County and surrounding areas while maintaining a balanced caseload and strong teamwork.

Qualifications

• Bachelor’s Degree from an accredited 4‑year college with a major in social work, psychology, sociology or a closely related field; OR Bachelor’s Degree in any field with one year of social work/case management experience.

Responsibilities

• Attend bi-annual meetings with clients to monitor services.
• Develop and maintain collaborative relationships with clients as a role model.
• Assess client needs and ensure continuity of care.
• Maintain regular contact with collaborative partners.
• Make referrals and authorize services based on client needs.

Education

Bachelor’s Degree in Social Work / Psychology / Sociology or related field

Bachelor’s Degree in any field + 1 year social work experience
