People Incorporated Mental Health Services is hiring a Case Manager I or II to join our Dakota County Targeted Case Management team. The role focuses on clients with chronic mental illness and other health conditions, developing person-centered plans and autonomous scheduling.
Responsibilities include functional assessments, plan reviews, and referrals, with monthly meetings to monitor progress. Qualifications include a behavioral science degree or RN with relevant psychiatric experience; II
Qualificações
- Bachelors in behavioral sciences or related field required; RN with relevant training accepted.
- For Case Manager II: 2,000 hours supervised experience or a Master's degree.
Responsabilidades
- Perform functional assessments leading to individualized plans with access to needed services.
- Regularly review plan effectiveness and adjust to maximize support.
- Assess client needs and ensure continuity of care.
- Make referrals based on assessment and current client needs.
- Maintain daily records and document interventions per standards.
- Attend monthly client meetings to monitor services.
