Job summary

Affinity Care MN is hiring a mission-driven Waiver Case Manager to provide person-centered case management for individuals with mental illness and co-occurring conditions in Hennepin County and surrounding areas. The role requires meeting clients regularly, coordinating with care teams, and ensuring proper authorizations and contracts are in place.

Qualified candidates will have a qualifying degree or equivalent experience, travel willingness, and strong communication skills.

Qualifications

• Must have one of the following: a bachelor’s degree in social work, psychology, sociology, or related field; OR a bachelor’s degree in any field plus one year of social work/case management experience; OR willingness to travel with a valid driver’s license and vehicle.
• Maintain acceptable motor vehicle record and be able to drive in Metro Area.
• Proficiency with office productivity software platforms.

Responsibilities

• Carry and maintain a caseload of individuals with serious mental illness, substance use disorders, and co-occurring conditions.
• Meet with clients monthly and twice yearly as required by waiver rules.
• Manage client intake and discharge within parameters and timelines.
• Collaborate with physicians and care teams to update each client’s plan.
• Ensure financial authorizations and provider contracts are in place.
• Document services in each client’s record for accessibility and outcomes tracking.
• Connect clients with community and housing resources.
• Provide crisis assessment and intervention to keep clients safe at home and in the community.
