Job summary

Paradigm in Massachusetts seeks a Vocational Case Manager to support Boston and surrounding areas with a hybrid schedule and travel requirements. You will assess disability-related work impacts, develop rehabilitation plans, and identify suitable employment opportunities.

Required CRC, CCM, or CDMS credentials, bachelor’s degree, and strong communication/organizational skills; master’s degree preferred.

Qualifications

• Bachelor’s degree in Rehabilitation Counseling or related field required.
• Master’s degree in Rehabilitation Counseling or advanced preferred.

Responsibilities

• Schedule appointments with clients, families, insurance adjusters, providers, and employers.
• Assist and coordinate return-to-work planning, job descriptions, and suitable employment opportunities.
• Coordinate with treatment providers to assess capabilities and readiness for return to work.
• Complete Vocational Assessments by analyzing medical, social, economic, education, and work history data.
• Develop Rehabilitation Plan/Case Management Plan tailored to client goals.
