Job summary

Ponos Care is a physician-led, value-based healthcare organization delivering holistic care to patients with chronic inflammatory and immune conditions. We seek an RN Case Manager to blend telephonic case management with travel-infused infusion therapy and RPM across multiple states, improving outcomes and reducing hospital utilization.

Responsibilities include comprehensive assessments, individualized care planning, infusion administration, and close collaboration with interdisciplinary teams.

Qualifications

• Bachelor's degree in nursing (BSN) and active RN license in states of practice.
• CPR/BLS certification required.
• Experience serving Medicaid or complex care populations is preferred.

Responsibilities

• Conduct comprehensive member assessments via telephonic outreach and in-home visits.
• Develop and implement individualized care plans across medical, behavioral health, and social needs.
• Provide infusion therapy and acute pain management in patient homes or community settings.
• Coordinate services across primary care, specialists, and community resources.
• Document all activities in the EHR and ensure CMS, NCQA, and HIPAA compliance.
• Support health equity efforts and adapt to remote/hybrid care delivery.
