Job summary

Ponos Care, a physician-led value-based health organization, seeks an RN Case Manager for a hybrid telephonic role supporting Medicaid populations across multiple states. This role blends remote case management with travel-based infusion therapy and LTSS responsibilities.

You will perform comprehensive assessments, create individualized care plans, coordinate services, and educate patients while ensuring regulatory compliance and high-quality outcomes.

Qualifications

• BSN degree and active RN license across required states.
• CPR/BLS certification required; CCM preferred.
• Experience with Medicaid or managed care populations is a plus.

Responsibilities

• Conduct telephonic assessments and in-home visits to plan care.
• Coordinate infusion therapy, RPM data, and inter professional care.
• Educate members and families on treatment plans and disease management.
• Ensure regulatory compliance (CMS, NCQA, HIPAA) in documentation.
