Job summary

Ponos Care is seeking an RN Case Manager for a hybrid/telephonic role that supports Medicaid populations across multiple states. The role combines telephonic case management with travel-based home infusion therapy, TOC, RPM, and LTSS responsibilities, coordinating care with interdisciplinary teams to reduce readmissions and improve outcomes.

The ideal candidate holds a BSN with an active multi-state RN license, 2+ years of clinical nursing experience, and CPR/BLS.

Qualifications

• Bachelor’s degree in nursing and an active RN license in all states of practice.
• Minimum 2+ years of clinical nursing experience, with care management or infusion exposure preferred.
• CPR/BLS required; CCM and/or infusion-specific certifications are a plus.
• Willingness to travel to patient homes across regions; strong communication skills.

Responsibilities

• Conduct comprehensive patient assessments via telephonic outreach or in-home visits as appropriate.
• Develop individualized care plans addressing medical, behavioral health, social determinants and infusion needs.
• Provide infusion therapies and acute pain management in home or community settings.
• Coordinate services across care teams and community resources to ensure smooth transitions.
• Document in EHR, ensure regulatory compliance and support quality improvement.
