Job summary

Ponos Care in the United States is seeking an RN Case Manager to support Medicaid populations through telephonic outreach, comprehensive assessments, and care coordination. This hybrid role combines remote case management with travel-based infusion therapy and LTSS responsibilities across multiple states.

Preferred candidates hold a BSN, an active multistate RN license, CPR/BLS, and 2+ years in clinical nursing or care management; CCM certification is a plus. Travel to patient homes is required.

Qualifications

• Education: BSN or equivalent required; active RN license in multiple states.
• Experience: 2+ years in clinical nursing or care management; Medicaid experience preferred.
• Certifications: CCM preferred; ACLS/ONCC/CRNI desirable.

Responsibilities

• Telephonic outreach and comprehensive member assessments.
• Develop and monitor personalized care plans and coordinate services.
• Administer infusion therapy at home and monitor patient responses.
• Document care activities and ensure regulatory compliance.
• Travel to patient homes across assigned regions for infusion therapies.
