Qualifications

• Requires bachelor's degree and RN license; social work path acceptable.
• Demonstrated knowledge of case management and discharge planning.
• Experience in acute hospital settings preferred.

Responsibilities

• Manage a focused caseload of 20–25 complex patients.
• Coordinate discharge plans with physicians, payers, and care teams.
• Collaborate with leaders and ensure regulatory compliance.
• Educate and support patients and families throughout care transition.
• Identify and resolve barriers to safe discharge.
