The Advanced Practice Provider (APP) will work closely with supervising Physicians and will manage 3-5 separate primary care micro clinics and care for the surrounding communities.

Qualifications

• Graduate of a Nurse Practitioner program approved by the State of New Mexico; Master of Nursing required.
• Licensure as an Acute Care NP and eligibility for prescriptive authority in NM or another state.
• DEA licensure eligibility; Basic Cardiac Life Support certification.

Responsibilities

• Provide medical services under physician supervision across multiple clinics.
• Perform physical exams, order/interprete diagnostics, document visits.
• Offer health coaching, disease management, and preventive screening.
• Utilize telemedicine for virtual visits and hybrid care delivery.
• Float across facilities and respond to urgent care needs.

Position Summary:

The Advanced Practice Provider (APP) will work closely with supervising Physicians and will manage 3-5 separate primary care micro clinics and care for the surrounding communities. It will consist of a combination of virtual and in-person medical care. The key objective is to create value through health coaching, disease management, preventative screening, and education.

Essential functions:

• Promotes and maintains patient health by providing medical services under the supervision of a physician.
• Contributes to a physician’s efficiency by identifying short-term and long-range patient care issues.
• Recommends options and courses of action for patient care.
• Implements physician directives.
• Assesses patient health by interviewing patients and performing physical examinations including obtaining, updating, and studying medical histories.
• Determines abnormal conditions by administering or ordering diagnostic tests, such as X-rays and laboratory studies and interpreting test results.
• Documents patient care services by charting in patient and department records.
• Performs therapeutic procedures by administering injections and immunizations, suturing, and managing wounds and infections.
• Instructs and counsels patients by providing normal growth and development information and providing counseling on emotional problems of daily living.
• Provides a community of care by developing and implementing patient-management plans.
• Maintains a safe and clean working environment by complying with procedures, rules, and regulations.
• Protects patients and employees by adhering to infection-control policies and protocols. Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; and enforcing adherence to requirements and advising management on needed actions
• Collaborates in the development and evaluation of patient care. Updates clinical guidelines/protocols/pathways; works autonomously and accepts accountability for outcomes; signs orders in accordance with institution and state board requirements; collaborates in research and teaching projects and applies research to practice.
• Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
• Develops health care team staff by providing information, educational opportunities, and experimental growth opportunities.
• Utilizes telemedicine interface to complete virtual visits.
• Ability to float, round to multiple facilities and/or take call (first call) to support Urgent Care APPs.

Requirements

Nurse Practitioner

Education:

Graduate of a Nurse Practitioner program approved by the State of New Mexico; Master of Nursing required.

Licenses/Certifications:

• Licensed to practice professional nursing as an Acute Care Nurse Practitioner by the State of New Mexico or other specified state (s).
• Licensure by the State of New Mexico APRN and eligibility for full prescriptive authority by the State of New Mexico or other specified state (s)
• Certified by a recognized national certification by the State Board of New Mexico or other specified state (s) in area of population foci and/or additional specialty recognition beyond the APRN.
• Eligibility for DEA licensure.
• Basic Cardiac Life Certification.
